经牙槽嵴顶提升术在上颌后牙种植修复中的应用效果

摘    要:目的 探讨经牙槽嵴顶提升术在上颌后牙种植修复中的应用效果。方法 选取我院2019年1月-2021年 1月收治的100例上颌后牙种植修复患者为研究对象,按照随机数字表法分为对照组和观察组,各50例。对 照组采用侧壁开窗上颌窦底提升术联合同期种植,观察组采用经牙槽嵴顶提升术联合同期种植,比较两组手 术指标、近远期疗效及疼痛程度。结果 两组上颌窦提升高度、种植体直径和种植体长度比较,差异无统计 学意义(P>0.05);观察组手术时间短于对照组,差异有统计学意义(P<0.05);两组术后12、24个月周围 骨吸收比较,差异无统计学意义(P>0.05);观察组术后12、24个月种植体稳定性系数、剩余牙槽骨高度 和平均获得骨高度均高于对照组,差异有统计学意义(P<0.05);观察组术后12、24 h VAS评分均低于对 照组,差异有统计学意义(P <0.05)。结论 采用经牙槽嵴顶提升术进行上颌后牙种植修复可有效缩短手 术时间,降低术后疼痛程度,改善植体稳定性系数、剩余牙槽骨高度和平均获得骨高度。

Application Effect of Transalveolar Maxillary Sinus Floor Elevation in Implant Restoration of Maxillary Posterior Teeth

Abstract:Objective To investigate the effect of transalveolar maxillary sinus floor elevation in implant restoration of maxillary posterior teeth. Methods A total of 100 patients with implant restoration of maxillary posterior teeth admitted to our hospital from January 2019 to January 2021 were selected as the research objects. According to the random number table method, they were divided into control group and observation group, with 50 patients in each group. The control group was treated withlateral window sinus floor elevation combined with simultaneous implantation, and the observation group was treated with transalveolar maxillary sinus floor elevation combined with simultaneous implantation. The surgical indicators, short-term and long-term efficacy and pain degree were compared between the two groups. Results There was no significant difference in the height of maxillary sinus elevation, implant diameter and implant length between the two groups (P >0.05). The operation time of the observation group was shorter than that of the control group, and the difference was statistically significant (P <0.05). There was no difference in the peripheral bone resorption data between the two groups 12 months and 24 months after surgery (P >0.05). The implant stability coefficient, residual alveolar bone height and average bone height of the observation group were higher than those of the control group at 12 and 24 months after operation, and the differences were statistically significant (P <0.05). The VAS score of the observation group at 12 and 24 h after operation was lower than that of the control group, and the differences were statistically significant (P <0.05). Conclusion Transalveolar maxillary sinus floor elevation can reduce operation time and postoperative pain, and improve implant stability coefficient, residual alveolar bone height and mean acquired bone height.
