影响臀位妊娠结局的因素分析

目的 探讨臀位妊娠情况下．影响母儿结局的因素及合理分娩方式的选择原则。方法 对单胎臀位妊娠369例的临床资料进行单因素及多因素分析，找出影响单胎臀位妊娠结局的因素。结果 剖宫产组新生儿需转NICU率、新生儿并发症率、围产儿死亡率均较阴道分娩组低（P〈0．05）。剖宫产组产妇产后出血率及产后并发症发生率均较阴道分娩组高（P〈0．01），足月妊娠度剖宫产对臀住妊娠的新生儿有利．而剖宫产对母亲来说则是不利因素。结论 臀位妊娠分娩方式的选择应全面考虑围产儿及母亲因素，结合产妇及家属的意愿来决定。

Analysis of tile in fluential factors on the outcome of breech presentation

Objective To analyze the factors affecting the maternal and neonatal outcomes for pregnant women of breech presentation and to explore the reasonable mode of delivery for breech presentation.Methods we collect the clinical data of total 369 cases of singleton breech presentation admitted in our hospital form January 1991 to December 2002 and compare these data between the group of vaginal delivery and the group of cesarean section.We also discuss the factors which have and effect on maternal and perinatal outcomes through univariate and multivariate analysis.Results Univariate analysis indicates that the rate of the newborns need to turn NICU,the newborn's complications rate and mortality was lower in the group of cesarean cection than that in the group of vaginal delivery.The perinatal outcomes between the two groups had significant diffecrence,P<0.05.But the rate of postpartum hemorrhage and puerperal morbidity is higher in the group of cesarean section than that in the group of vaginal delivery.There was significant difference in the maternal outcomes between the two groups.P<0.01.Multivariate analysis indicates that term pregnancy and cesarean section are favorable factors for the newborns,But CS had an adverse effect on the maternal outcomes.Conclusion When we determine the mode of delivery for breech presentation,we must take all into account the maternal factors,the perinatal factors as well as the will of the patients and their relatives.
