摘    要:【目的】探讨无创机械通气联合丙种球蛋白、甲泼尼龙治疗重症手足口病的疗效。【方法】回顾性分析2009年3月至2012年3月重症手足口病患儿326例,其中未使用无创机械通气者159例(A组),使用无创机械通气者167例(B组)。比较观察两组的生命体征恢复至正常所需时间及临床治疗的有效率。【结果】B组患儿体温、呼吸、心率、血压恢复至正常的时间均显著短于A组(P〈0.01)。A组的治疗总有效率为88.1%,显著低于B组的94.6%(P〈0.05)。【结论】在常规综合治疗的基础上早期联合使用无创机械通气治疗重症手足口病可有效的阻断疾病的发展,显著提高临床疗效,其无创性及有效性易为患方接受。

Abstract:Objective] To explore the efficacy of noninvasive mechanical ventilation combined gamma globulin and methylprednisolone for the treatment of severe hand-food-mouth disease(HFMD). Methods] A total of 326 pediatric patients with severe HFMD from March 2009 to March 2012 was analyzed retrospective- ly. Among them, 159 patients(group A) didn't use noninvasive mechanical ventilation, and 167 patients(group B) were treated with non-invasive mechanical ventilation. The recovery time of vital signs and clinical efficien- cy were compared between two groups. Results] The temperature, breathing, heart rate and the recovery time of blood pressure in group B were significantly lower than those in group A( P 〈0.01). The total effec- tive rate in group A was 88.1%, which was significantly lower than that in group B(94.6%)( P 〈0.05). Conclusion] Early noninvasive mechanical ventilation based on the routine treatment can be effectively block the development of severe HFMD, and significantly improve the clinical efficacy. Its non-invasion and validity may be easily accepted by patients.
