高容量血液滤过治疗老年危重病合并高钠血症临床观察

【目的】观察高容量血液滤过（HVHF）治疗老年危重病合并高钠血症的疗效。【方法】对30例老年危重病合并高钠血症患者行HVHF治疗，治疗中动态监测血清钠以调整置换液钠浓度，观察HVHF治疗前后患者生命体征、血清钠、血流动力学的变化。【结果130例患者治疗后血清钠、体温下降，氧合指数较治疗前升高，且差异有显著性（均P〈0．05），心率、平均动脉压较治疗前无变化（均P〉0．05）。治疗过程中患者血流动力学稳定，安全性好。【结论】HVHF是治疗老年危重病合并高钠血症新的有效途径。

Clinical Observation on High Volume Hemofiltration for the Treatment of Elderly Critical Dis-ease with Hypernatremia

[Objective]To observe the clinical efficacy of high volume hemofiltration（HVHF） for the treat- ment of elderly critical disease with hypernatremia. [Methods]Thirty elderly critical patients with hypernatre- mia were treated with HVHF. Serum sodium was dynamically monitored to adjust sodium concentration in exchange fluid. Vital signs, serum sodium concentration, oxygenation index and hemodynamic change of patients before and after HVHF were observed. [Results]Compared with before treatment, body temperature and ser- um sodium concentration of 30 patients after treatment decreased and the oxygenation index increased, and there was significant difference（all P 〈0.05）. But heart rate and mean arterial pressure had no change in com- parison with before treatment（all P 〈0.05）. During the treatment, the hemodynamics of the patients was sta ble and the treatment was safe. [Conclusion]HVHF is a new and effective method in the treatment of elderly critical disease with hypernatremia.
