摘    要:目的:用实验动物模型同时切除肝脏、胰腺或十二指肠,以探讨残肝再生反应。方法:60只SD大鼠被随机分为4组:(1)68%肝叶切除组;(2)68%肝叶切加50%胰腺切除组;(3)68%肝叶切除加近端十二指肠切除组;(4)68%肝切除加50%胰腺切除加近端十二指肠切除组。每组中3只大鼠分别于术后12、24、48、72和168h处死并采集血液和肝脏样本,计算肝再生率以及免疫组化证实肝细胞增殖细胞核蛋白抗原(PCNA)的表达。结果:肝、胰切除和肝、胰十二指肠切除可显著减少残肝细胞DNA峰值期的合成,其延迟性肝再生反应延迟至术后168h,结论:起源于胰腺和十二指肠外分泌腺的多种因子对触发肝细胞的增殖可能起显著作用。

Effect of Hepatopancreatoduodenectomy on Hepatic Regeneration in Rats

Abstract:Objective:To explore the hepatic regeneration responses following stimultaneous resection of the hepar and the pancreas and the duodenum in an experimental animal model.Methods: Sixty SD rats were divided into four groups: 68% Hepatectomy group (Hx, n =15); Hx plus 50% partial pancreatectomy group (Hx plus Px, n =15); Hx plus duodenectomy group (Hx plus Du, n =15) and HPD group( n =15). Three rats in each group were sacrificed for blood and hepatic collection on 12,24,48,72 and 168 postoperative hours, respectively. PCNA labeled nuclei expression in the hepar was demonstrated immunohistochemically and the hepatic regeneration rate(LRR) was calculated. Results: The highest peak of PCNA expression of hepatocytes in the remaining lobes were markely reduced both on 24 hour after Hx plus Px and HPD, with a delay in the hepatic regeneration responses extending to 168 postoperative hour. Conclusion: Multifactors originated from the exocrine glands of the pancreas and the duodenum may play a distinct role in triggering hepatocyte proliferation.
