摘    要:目的 探讨低分子右旋糖酐和丹参注射液对经皮冠脉介入(PCI)治疗后造影剂肾损伤的保护作用和预防肾功能损伤的效果.方法 将行PCI术的120例患者随机分为低分子右旋糖酐和丹参注射液治疗组与对照组各60例,治疗组在常规治疗基础上加用低分子右旋糖酐250 ml和丹参注射液16 ml应用7d.两组均在治疗前及术后第1天、第6天检测血尿素氮、肌酐、β2微球蛋白及24h蛋白尿.结果 两组造影剂应用后第1天血尿素氮、肌酐、β2微球蛋白及24h蛋白尿均升高,第6天恢复至基线水平,治疗组明显低于对照组(均P<0.05).结论 围手术期应用低分子右旋糖酐和丹参注射液治疗可有效减轻造影剂对肾脏的损害,减低肾功能不全的发生率.

Abstract:Objective To explore the protective effect of low molecular dextran combined with salvia miltiorrhiza injection against kidney damage from contrast agent after percutaneous coronary intervention(PCI) and the effect of preventing kidney damage.Methods 120 patients who underwent PCI were randomly divided into two groups:the treatment group(low molecular dextran combined with salvia miltiorrhiza treatment) and the control group,each group 60 cases.The control group was given the conventional treatment,and the treatment group was treated with 250ml low molecular dextran and 16ml salvia miltiorrhiza injection for 7d on the basis of conventional treatment.The levels of blood urea nitrogen(BUN),serum creatinine(SCR),β2 microglobulin(β2-MG),24h urine protein were detected before and 1d,6d after surgery.Results At one day after application of contrast agent,the levels of BUN,SCR,β2-MG,24h urine protein were increased,and returned to baseline level at 6th day.The levels of BUN,SCR,β2-MG,24h urine protein of the treatment group were significantly lower than those of the control group at 6th day(P<0.05).Conclusion The low molecular dextran combined with danshen injection treatment in the perioperative period could effectively reduce the kidney impairment from contrast agent and the incidence of renal insufficiency.
