胺碘酮治疗电击伤并发心房颤动的临床分析

目的 观察静脉应用胺碘酮对电击伤并发心房颤动的疗效和安全性.方法 12例电击伤患者,心电图证实为心房颤动.在常规治疗的基础上予胺碘酮150 mg静脉注射,继之用微量泵以0.5～1.5mg/min维持泵人,持续24h.观察心律转复情况、心室率、血压、QT间期及其不良反应.结果 12例患者均成功转复为窦性心律,转复成功率100％.平均转复时间为（285±75）min.心房纤颤转复为窦性心律前后心室率变化有统计学意义（P＜0.01）;动脉收缩压、舒张压差异无统计学意义（P＞0.05）;在用药后24 hQT间期变化有统计学意义（P＜0.05）.无严重不良反应.结论 静脉应用胺碘酮治疗电击伤并发的心房颤动是安全、有效的方法.

Clinical analysis of amiodarone in the treatment of electric injury complicated by atrial fibrillation

Objective To evaluate the efficacy and safety of intravenous injection amiodarone for electric injury complicated by atrial fibrillation. Methods 12 patients with electric injury had atrial fibrillation evidenced by ECG. In addition to routine treatment, amiodarone （150 mg） was given by intravenous injection, followed by a intravenous maintenance dose of 0.5-1.5 mg/rsin for 24 hours. Cardioversion of atrial fibrillation, ventricular rate, blood pressure, QT interval and adverse reactions were observed. Results 12 patients were all successfully converted to sinus rhythm, the successful rate of conversion was 100%. The average reversal time was （285 ＋ 75）rsin. After returned to sinus rhythm, the ventricular rate change has the remarkable statistical significance （P〈0.01）;the artery systolic pressure, the diastolic pressure have nonstatistieal difference （P〉0.05）; at the time of 24 hours after administration, QT interval change has the remarkable statistical significance （P〈0.05）. No serious adverse reaction occurred. Conclusion Intravenous Amiodamne is safe and effective in management of electric injury corsplieated by atrial fibrillation.
