中西医结合疗法治疗慢性阻塞性肺疾病急性加重合并呼吸衰竭临床疗效及安全性的Meta分析

目的 分析中西结合疗法用于慢性阻塞性肺病（COPD）急性加重合并呼吸衰竭临床治疗中的效果与安全性.方法 通过计算机检索Cochrance、OVID、Pubmed图书馆数据库相关信息资料,查找单纯西医治疗与中西医治疗慢性阻塞性肺病急性加重合并呼吸衰竭的随机对照临床实践.用数据RevMan 5.1软件对筛选出的研究进行Meta分析,研究中西医结合临床效果和安全性.结果 共纳入10篇文献502例患者,研究组与对照组的病死率分别为6.97％、19.57％,OR=0.31 （95％CI=0.18～0.56）、P＜0.05.研究组与对照组的VAP患病率分别为6.56％、42.55％,OR=0.11 （95％CI=0.07～0.19）、P＜0.05;WMD（有创通气时间）=11.44（95％CI=14.45～8.38）、P＜0.05.结论 中西医结合用于COPD急性加重合并呼吸衰竭临床治疗相比单纯西医治疗更能有效降低VAP发病率、病死率并缩短有创通气时间,效果显著.

Analysis of the efficacy and safety of integrated traditional and western medicine in the treatment of chronic obstructive pulmonary disease combined with respiratory failure

Objective To analyze the Traditional and Western medicine therapy forchronic obstructive pulmonary disease and respiratory failure in clinical treatment effect. Methods Cochrance, OVID, Pubmed database libraries bycomputer, respiratory failure of chronic obstructive pulmonary disease searchpractice of Western medicine and traditional Chinese medicine and Western medicine therapy. Results Of the 10 articles included 502 patients, the mortalityrate of the study group and the control group were 6.97%, 19.57%, OR=0.31（95%CI=0.18~0.56）, P〈0.05; The prevalence rate of VAP were 6.56%, 42.55%,OR=0.11 （95%CI=0.07~0.19）, P〈 0.05 ;WMD （invasive mechanical ventilationtime） = 11.44 （95%CI=14.45~8.38）, P〈 0.05. Conclusion Significant for clinical treatment of COPD complicated with respiratory failure with combination of TCM and Western medicine.
