| 摘 要： | 目的：评价多种ＭＲＩ序列对髌骨软骨病诊断的敏感性，特异性和准确率，提出其中的优选序列。材料和方法：对５０例患者５１个髌骨进行术前ＭＲＩ检查，男性２３例，女性２７例２８个髌骨，其中３７例３８个髌骨行关节镜检查，１３例行手术治疗，以关节镜或手术结果为金标准行双盲前瞻性分析。结果：对髌骨软骨病的诊断，ＴＳＥ－Ｔ１Ｗ，ＴＳＥ－Ｔ２Ｗ，ＳＴＩＲ，Ｔ１－ＦＦＥ三个序列的敏感性分别是：５０％，４１３％，４５５％，８４６％，特异性分别是：８８２３％，９８％，９８１％，７４５％，准确率分别是：６２７％，６２０９％，６２７％，８１％。结论：Ｔ１－ＦＦＥ是三个序列中的优选序列。ＭＲＩ是诊断髌骨软骨病最优选影像学方法。

| Abstract: | PURPOSE:To evaluate the sensitivity,specificity and accuracy of multiple MR pulse sequence in diagnosing chondropathy patellae for sorting out the optimal pulse sequence. MATERIALS AND METHODS:This study covered 51 patellae of 50 patients. MR Imaging was performed for them befere operation. Among them 23 were males and 27 were females with 28 patellae. Arthroscopy was used in 38 knees,while the rest underwent arthrotomy. The gold standards were the results of arthroscopy or arthrostomy. The methods of analysis were two blinds and prespective. RESULTS:For the diagnosis of chondropathy patellae, the sensitivity of TSE-T 1W,TSE-T2W,STIR,T1-FFEwas 50%,41 1%,45 5%,84 6% respectively. The specificity was 88 23%,98%,98 1%,74 5% respectively. The accuracy was 62 7%,62 09%,62 7%,81% respectively. CONCLUSION: T1-FFE was the optimal sequence in diagnosing chondropathy patellae. MRI was one of the best imaging methods. |
