掌背动脉皮瓣修复手指皮肤缺损38例疗效观察

摘    要:目的:设计应用掌背动脉皮瓣修复手指掌面或背面的皮肤缺损,介绍一种手术方法,探讨其可行性与科学性。方法:在手背设计掌背动脉皮瓣逆行修复手指近节掌面或中节及近节背面的皮肤缺损,后期指导患者进行功能锻炼,自1996年至今应用38例(共43个皮瓣)。结果:43例皮瓣中成活42例,成功率97.6%,术后随访3个月至6年,手指的外观及运动和感觉功能均比较满意,按Swanson评定标准:手失能<5%。结论:掌背动脉皮瓣用于修复手旨皮肤缺损,可以缩短病程,便于早期进行功能锻炼,对于患者手指功能恢复及外观具有相当大的实用价值。

Abstract:Objective To use dorsal metacarpal arteries flaps in restoring palmar or dorsal cutaneous deficiency of fingers.To recommend a kind of operation method and study its practicability and scientific degree.Methods To design dorsal metacarpal arteries flaps on the back of the hand,then to instruct patients to finish functional exercise.We have finished 38 cases since 1996 (total 43 skin flaps).Results These flaps survived in 42 cases and the rate of success was 97.6%.The period of following up was 3 to 72 months after operations.The appearance and function of fingers were satisfactory.According to Swanson's standard the cheiro disability was less 5%.Conclusion It may shorten the course of disease and facilitate earlier functional training to use dorsal metacarpal arteries flaps in restoring cutaneous deficiency of fingers.It is a very valuable method for patients in their appearance and functional recover of fingers.
