韧带样型纤维瘤病的超声表现与病理特征

摘    要:目的分析超声诊断韧带样型纤维瘤病(DTF)的价值。方法回顾分析61例经手术病理证实的DTF患者的超声表现,并与病理表现进行对照。结果 DTF按病变部位分为腹内型(n=10)、腹壁型(n=14)及腹壁外型(n=37);超声表现均为深部肌层的弱回声肿块,3种类型血供均不丰富。超声诊断腹内型、腹壁型及腹壁外型DTF的符合率分别为70.00%(7/10)、85.71%(12/14)及86.49%(32/37)。腹内型DTF的最大径大于腹壁型(P=0.042),腹壁外型DTF内部回声更倾向于不均匀。结论 DTF的超声表现与病理学特征具有一定相关性,肿瘤内部出现条索状、结节状稍强回声区可能提示纤维成分增多。

Abstract:Objective To explore the value of ultrasonography in diagnosis of desmoid type fibromatosis (DTF) as well as corresponding of ultrasound performance to pathological features. Methods The ultrasonic characteristics of 61 DTF patients confirmed with pathology were analyzed retrospectively and compared with pathological features. Results According to the lesion position, DTF was classified as intraabdominal (n=10), in abdominal wall (n=14) or extraabdominal categories (n=37). Ultrasonogram showed hypoechoic masses in deep muscle, and the blood supply of all three categories was hypo-vascular. The diagnostic accordance rate of ultrasound was 70.00% (7/10), 85.71% (12/14) and 86.49% (32/37), respectively. The maximum diameter of intraabdominal DTF was bigger than that of abdominal wall DTF (P=0.042), and the echogenecity of extraabdominal DTF tended to be more heterogenous. Conclusion Ultrasonic performances of DTF have certain correlation with pathological features, and the stripped or nodular lesion which is hyperechoic in ultrasonogram probably prompts increasing of collagen fibers.
