摘    要:目的运用左心房容积追踪(LAVT)技术和实时三维超声心动图(RT-3DE)评价肥厚性心肌病(HCM)患者左心房功能。方法选取HCM患者31例(HCM组)和健康志愿者27名(对照组),运用LAVT技术获取左心房排空率曲线,测量收缩期左心房排空率峰值(dv/dts)、舒张早期左心房排空率峰值(dv/dtE)、舒张晚期左心房排空率峰值(dv/dtA);以RT-3DE测量左心房最大容积(LAVmax)、左心房最小容积(LAVmin)、左心房收缩前容积(LAVp)、左心房被动射血分数(LAPEF)、左心房主动射血分数(LAAEF),并对以上各参数进行对比分析。结果与对照组相比,HCM组dv/dts、dv/dtA增大而dv/dtE减小(P均<0.05),LAVmax、LAAEF增大而LAPEF减小(P均<0.05)。结论联合应用LAVT和RT-3DE可从不同角度综合评价HCM患者左心房功能。

Abstract:Objective To evaluate the left atrial (LA) function in patients with hypertrophic cardiomyopathy (HCM) with left atrial volume tracking (LAVT) technology and real-time three-dimensional echocardiography (RT-3DE). Methods Totally 31 patients with HCM (HCM group) and 27 control subjects (control group) were involved. Left emptying fraction was acquired from LAVT technology, systolic LA filling peak (dv/dts), early diastolic LA emptying peak (dv/dtE), late diastolic LA emptying peak (dv/dtA) were measured and analyzed. LA maximal volume (LAVmax), LA minimal volume (LAVmin), LA presystolic volume (LAVp), LA passive emptying fraction (LAPEF) and LA active emptying fraction (LAAEF) were measured with RT-3DE. All parameters were analyzed between the two groups. Results Compared with the control group, dv/dts, dv/dtA increased, dv/dtE decreased (all P<0.05), while LAVmax and LAAEF increased, LAPEF decreased (all P<0.05) in HCM group. Conclusion The combination of LAVT technology and RT-3DE can be used for evaluating LA function accurately in different point of view.
