椎-基底动脉供血不足性眩晕ENG和ABR检测的意义

摘    要:目的 探讨椎-基底动脉供血不足性眩晕患者眼震电图(ENG)和听性脑干反应(ABR)的改变及其临床意义.方法 对60例椎-基底动脉供血不足性眩晕患者行眼震电图和听性脑干反应检测,观察视眼动系统反应、自发性眼震、冷热试验、位置性眼震和ABR各参数变化.结果 椎-基底动脉供血不足性眩晕组位置性眼震强度(7.76±6.05)°/s,明显高于对照组(P<0.01).视动中枢检查共48例(75%)出现一项或多项异常.冷热试验总慢相速度为(34.68±5.66)°/s,对照组中总慢相速度为(46.57±6.34)°/s(P <0.01).听性脑干反应49例异常,占81.6%.脑干下部异常者25例(41.6%),脑干上部异常者12例(20%).结论 眼震电图和听性脑干反应检查可从电生理方面有效鉴别椎-基底动脉供血不足性眩晕患者的脑干与视眼动系统异常.尤其是对内耳与脑干功能损害的鉴别,ENG和ABR可提供客观有效的帮助.

Abstract:【Objective】 To explore the clinical relationship between Electronystagmography(ENG),Auditory brain-stem response(ABR) and patients with Vertebro-basilar insufficiency(VBI).【Methods】 60 patients with Vertebro-basilar insufficiency were selected as experiment group and 40 normal persons as control group.The differences of Electronystagmography and Auditory brain-stem response for both groups were recorded.【Results】 The incidence were 18.3% for spontaneous nystagmus test,43.3% of who had Ⅲ and Ⅳ type figure in the eyetracking test.The value of caloric test in the patients was lower than normal subjects.The frequency differences and the slow phase velocity differences of positional nystagmus were significantly greater in patients with Vertebro-basilar insufficiency compared with control group(P <0.01).There are 49(81.6%) abnormal ABR cases in VBI group.Of them,25(41.6%) cases the lower part of the brain stem is abnormal,12(20%) cases the supper part of the brain stem is abnormal.【Conclusion】 These results suggest that Electronystagmography and Auditory brain-stem response could be used as a objective test for diagnosis in Vertebro-basilar insufficiency.
