某化工厂3例职业性皮肤病的调查

目的调查某化工厂(生产左羟丙哌嗪)3例职业性皮肤病即"硬皮病"样改变的发生的原因。方法根据现场职业卫生调查情况、患者的病历资料及职业病诊断原则进行综合分析。结果病例均发生在产品左羟丙哌嗪预试验期间,因接触大量的苯胺及其化合物导致发生职业性硬皮病样综合征,病程迁延,彻底脱离致病环境者病情好转,严重者呈不可逆性倾向。正式投产运行后职业卫生管理完善,自动化程序提高,防护措施加强,未发生新的病例。结论结合患者的职业接触史及车间发病的情况和临床表现,并参考相关的资料,可以确认该化工厂发生的硬皮病样改变为苯胺及其化合物所致的职业性皮肤病——硬皮病。

Investigation on 3 cases of occupational dermatosis in a chemical plant

Objective]To investigate the causes of 3 cases of occupational dermatosis scleroderma-like changes in a chemical plant(manufacturing levodropropizine).Methods]The comprehensive analysis was implemented according to the field occupational health survey results,the medical records of patients and the diagnosis principles of occupational disease.Results] All cases of occupational scleroderma-like syndrome appeared at the pre-trial period of levodropropizine production,due to exposure to aniline and its compounds in large amount.The disease course was prolonged,the patients leaving pathogenic environment was improved,while the serious patients showed irreversible tendency.The occupational health management,automated procedures,protective measures had been improved and no new case had been reported in this factory,since put into production formally.Conclusion]The cases with scleroderma-like syndrome is occupational dermatosis scleroderma caused by aniline and its compounds in this factory according to occupational exposure history of patients,the workshop incidence and clinical manifestation as well as relevant information.
