改良标准大骨瓣开颅术在重型颅脑损伤中的应用

目的探讨改良标准大骨瓣开颅术治疗重型颅脑损伤的疗效。方法分别采用改良标准大骨瓣开颅术与经典开颅术治疗重型颅脑损伤的患者140例,将其分为改良标准大骨瓣组（治疗组n=68例）和经典开颅组（对照组n=72例）,采用格拉斯哥预后（GOS）评分比较两组术后3个月的愈后。结果术后GOS评分,治疗组68例中,4-5分30例（44.1%）,2-3分21例（30.9%）,1分17例（25.0%）,对照组72例中,4-5分20例（27.8%）,2-3分23例（31.9%）,1分29例（40.3%）。GOS 1分及4-5分两者相比较,差异有统计学意义（P〈0.05）。结论在重型颅脑损伤患者,行改良标准大骨瓣术者预后较优于经典骨瓣开颅术者。

Clinical Application of Improve Stand Large Trauma Craniotomy on Severe Craniocerebral injury

Objective To explore the curative effect of improved stand large trauma craniotomy in serve head injury.Methods A total of 140 patients with severe craniocerebral injury were divided into two groups;68 patients in the treatment group were treated by improved large trauma craniotomy and another patients in control group were received routine treatment.The effective cure was compared in two groups according to Glasgow outcome scale（GOS） after 3 month treatment.Results Based on GOS,there was significant difference in effective assay between the two group patients.Conclusion The patients treated by improve large trauma craniotomy have better advantages than those done by the routine operation.
