恶性肿瘤患者凝血指标临床分析及其与血瘀证的相关性

目的 探讨凝血指标在不同瘤种、不同分期的恶性肿瘤患者中分布特点及其与中医血瘀证的相关性.方法 通过回顾性调查,收集明确诊断为肺癌、胃癌、大肠癌、乳腺癌患者的临床资料,对不同瘤种、不同分期患者凝血指标及血瘀证进行比较分析.结果 共收集合格病例351例,其中共有343例凝血异常占97.72％.与早期比较,晚期肺癌及胃癌纤维蛋白原(FIB)的升高分度明显增高(P＜0.05);与大肠癌、乳腺癌、胃癌比较,肺癌患者的FIB升高分度明显增高,以Ⅲ度为主(P＜0.05或P＜0.01).与早期比较,晚期肺癌D-二聚体(D-D)升高者分度明显增高(P＜0.05)；晚期各瘤种比较,与乳腺癌、胃癌比较,肺癌患者的D-D升高分度明显增高,以Ⅲ度为主(P＜0.01).与早期比较,晚期乳腺癌患者血瘀比例增高,且血瘀证分级明显增高(P＜0.05).晚期与大肠癌和胃癌比较,肺癌及乳腺癌血瘀证分级明显增高(P＜0.05).Logistic回归分析显示,血瘀证与FIB、纤维蛋白原降解产物(FDP)升高存在相关性(P＜0.05).结论 恶性肿瘤患者普遍存在凝血异常,多数患者合并有“血瘀”证,其中,肺癌分期越晚高凝状态越重、血瘀程度越重;血瘀证与凝血指标FIB、FDP升高存在相关性.
