血糖与急性心肌梗死预后相关性研究

目的探讨急性心肌梗死(AMI)患者早期血糖升高与AMI近期预后的相关性。方法选择我院1995—2002年65例确诊的非糖尿病AMI患者,按空腹血糖值分组,≤6.4mmol/L为血糖正常组,>6.4 mmol/L为血糖升高组,比较2组患者的肌酸磷酸激酶、乳酸脱氢酶、谷草转氨酶、血脂水平和心力衰竭、心源性休克、心律失常发生率及近期病死率。结果血糖升高组的心肌酶等明显高于血糖正常组(P<0.05),2组间血脂水平无显著性差异(P>0.05),血糖升高组心力衰竭、心源性休克、心律失常发生率和病死率明显高于血糖正常组,有显著性差异(P均<0.05)。结论AMI早期血糖升高与AMI的预后不良有关。

Relationship study between blood sugar and prognosis of acute myocardial infarction

Objective It is to study the relationship between blood sugar raise and the early prognosis of acute myocardial infarction(AMI).Methods 65 final diagnosis non-diabetic AMI patients in our hospital from 1995 to 2002 were selected.They were divided into two groups according to fasting blood sugar.The patient whose blood sugar was ≤6.4 mmol/L was in blood sugar normal group and the patient whose blood sugar was >6.4 mmol/L was in blood sugar heightening group.The levels of creatine phosphokinase,lactic dehydrogenase(LDH),glutamic oxaloacetic transaminase(GOT) and blood lipid and the complication genesis rate and recent mortality rate of heart failure,cardiogenic shock and arrhythmia were compared.Results The cardiac muscle enzyme in blood sugar heightening group was much higher than that in blood sugar normal group(P<0.05).There was not significantly different on blood lipid level between the two groups(P>0.05).The complication genesis rate and mortality rate of heart failure,cardiogenic shock and arrhythmia in blood sugar heightening group were much higher than those in blood sugar normal group,and there was significant difference(P<0.05).Conclusion Early blood sugar heightening of AMI patients may be related to bad prognosis of AMI.
