不稳定型心绞痛患者血清脂蛋白（a）水平的变化

目的观察不稳定型心绞痛（UAP）患者血清脂蛋白（a）（Lp（a）1水平的变化及其意义。方法89例经冠脉造影证实的心绞痛患者，其中UAP患者68例，根据血清心肌肌钙蛋白I（cTnI）水平分为低危组（〈1．0ng／mL）和高危组（≥1．0ng／mL）。21例稳定型心绞痛（SAP）患者设为对照组。检测血清Lp（a）和cTnI的水平，观察Lp（a）与cTnI水平之间的相关性。结果UAP组患者血清cTnI和Lp（a）均高于SAP组（P〈0．051，高危组UAP患者血清Lp（a）水平高于低危组（P〈0．001），Lp（a）与cTnI有显著的相关性（r2=0．4027，P〈O．0001）。结论Lp（a1水平升高是高危UAP的危险因素，与心肌损伤有关。

Levels of serum lipoprotein (a) in patients with unstable angina pectoris

Objective To measure serum lipoprotein （a） Lp（a）] levels in patients with unstable angina pectoris （UAP） and to investigate its significance. Methods 89 patients with angina pectoris were diagnosed by angiography. 21 patients with stable angina pectoris （SAP） were recruited as control group. Venous blood samples were collected from 68 patients with UAP to measure serum Lp（a） levels on admission and serum cardiac troponin I （cTnI） levels in 12, 24, 48 hours and 5 days after admission. 35 patients with serum cTnI levels 〈1.0 ng/mL were assigned as low risk unstable angina group and 33 patients with serum cTnI levels ≥1.0 ng/mL were assigned as high risk unstable angina groups. And the relation of Lp（a） and eTnI was investigated. Results The levels of eTnI and Lp（a） in UAP were significantly higher than those of SAP （P 〈0.05）. Compared with low risk UAP, Lp（a） levels were significantly higher in high risk UAP （P 〈0.001）. There was a significant correlation between Lp（a） and eTnI levels （r2=0.402 7, P 〈0.0001）. Conclusion Increased Lp（a） levels and significant relation between Lp（a） and eTnI levels support the idea that Lp（a） can be a risk factor in patients with high risk UAP. High levels of Lp（a） may be related with myocardial injury in patients with UAP.
