胃大部切除术后食管癌的外科治疗

目的探讨胃切除术后食管癌的外科手术治疗.方法通过我科的9例手术,再结合文献分析胃大部切除术后食管癌手术切除重建的各种术式方法及优缺点.结果中上段食管癌切除应以横结肠代食管重建为佳,下段食管癌以残胃代食管重建最理想,残胃贲门癌可以用残胃或空肠代食管重建.结论胃大部切除术后食管癌是可以通过手术再根治的,食管的重建以残胃最接近生理及解剖学,术式简单,安全,易行.

Surgical treatment esophageal carcinoma in patient who had had partial gastreatomy

Aim To study the modality of surgical treatment of esophageal carcinoma in patients who had had gastreatomy. Methods By surgical treatment of 9 esophageal carcinoma in patients who had had gastreatomy integrated with literatures, the modalities of reconstruction of esophagus are analyed. Results Reconstruction of esophagus with colon is safe and effective in esophageal cancer of upper and middle segment. Reconstruction of esophagus with the residual stomach is the most idea in esophageal cancer of lower segment.Reconstruction of esophagus with residual stomach or jejunum may be implemented in the cancer of residual stomach and cardia. Conclusion The esophageal carcinoma can be operated in patients who had had gastreatomy.Reconstrution of esophagus with residual stomach accords with human physiology and anatomy,this modality of surgery is handy,little damage and has less post-operative complications.
