超声造影及灰阶弹性成像在肾透明细胞癌诊断中的应用

摘    要:目的 探讨超声造影及超声灰阶弹性成像技术对肾透明细胞癌的诊断价值.方法 回顾性分析20例经手术病理证实的肾透明细胞癌常规超声、超声造影及灰阶弹性成像检查结果,对以上3种检查结果比较分析.结果 20例肾透明细胞癌超声造影及灰阶弹性成像能清晰显示肿瘤边界,且显示的肿瘤最大径大于常规超声检测值;彩色多普勒超声显示3例肿瘤血流信号丰富,超声造影则显示19例肿瘤为富血供.超声造影增强模式12例为快进慢退高增强,余增强方式不一;超声造影显示瘤内坏死区及假包膜效果优于常规超声(P=0.004,P=0.039).灰阶弹性成像显示肿瘤内部回声不均匀,18例白色为主,2例黑色为主.结论 超声造影及灰阶弹性成像可为临床提供更多信息,从而对肾透明细胞癌的诊断提供更多帮助.

Abstract:Objetcive To investigate the diagnostic value of contrast - enhanced ultrasonography (CEUS) and gray - scale ultrasound elastography ( g - UE) in clear cell renal cell carcinoma (CCRCC). Methods The findings by conventional color Doppler ultrasonography (US) , CEUS and g- UE of 20 cases of CCRCC confirmed by surgical pathology were retrospectively compared and analyzed. Results CEUS and g - UE images of 20 cases of CCRCC revealed distinct tumor contours and greater parameters in diameter of primary tumors as compared to conventional US. Conventional US revealed only 3 cases with abundant tumor blood flow, while CEUS showed 19 cases. In the enhancement mode, 12 cases of CCRCC were in the mode of swift enhancement, slow expurgation and elevated enhancement, while the other cases showed heterogeneity; CEUS was superior to conventional US in revealing intratumor necrotic areas and psuedocapsules (P = 0. 004,P = 0.039 ). The g - UE demonstrated inhomogeneons echoes of tumors, with predominantly white images in 18 cases and predominantly black images in 2 cases. Conclusion CEUS and g - UE offer more information for clinical practitioners on, which facilitates clinical diagnosis of CCRCC.
