原发性胆囊癌的早期诊断及相关对策

目的　探讨原发性胆囊癌的早期诊断及对策。方法　回顾分析 64例原发性胆囊癌病人的临床病理资料及手术治疗情况。结果　 64例原发性胆囊癌病人有胆道疾病史者 47例 ,占 73 .4% ;首发症状以右上腹痛最常见 ,共 48例 ( 75 % ) ,其次为黄疸 2 1例( 3 2 .8% ) ,右上腹包块 14例 ( 2 1.9% ) ,发热 11例 ( 17.2 % )。术前诊断胆囊癌 41例 ,诊断率 64 .1% ( 4 1/64 ) ,B超和CT诊断率分别为72 % ( 3 6/5 0 )和 82 .4% ( 2 8/3 4)。结论　提高对原发性胆囊癌发病的认识 ,加强对高危人群的定期监测随访 ,联合运用B超和CT检查可明显提高本病的早期诊断 ,改善预后。

Early Diagnosis and Relevant Strategy On Primary Gallbladder Carcinoma

Objective To explore the early diagnosis and relevant strategy on primary gallbladder carcinoma (PGC) .Methods The clinical pathologic data of 64 patients with PGC were analyzed retrospectively.Results 47 of 64(73.4%) patients had the history of gallbladder diseases. Right epigastralgia was the most common initial symptoms, accounting for 75%(48/64), the others were jaundice (32.8%,21/64), right epigastric mass (21.9%,14/64), Fever (17.2%,21/64). 41 cases were diagnosed preoperatively, its diagnostic rate was 64.1%(41/64). Diagnostic rates of BUS and CT were 72%(36/50) and 82.4%(28/34) respectively.Conclusions The early diagnosis and prognosis for PGC were improved apparently through increasing recognition, strengthening surveillance and follow-up, and combining BUS with CT.
