NGAL、MCP-1 水平对重症肺炎的 诊断效能研究

摘    要:目的 探讨中性粒细胞明胶酶蛋白(NGAL)及血清单核细胞趋化蛋白1(MCP-1)对重症肺 炎的诊断价值。方法 选取普通社区获得性肺炎患者40 例(普通肺炎组),重症肺炎患者49 例(重症肺炎 组)及同期健康体检者55 例(对照组),测定3 组NGAL、MCP-1 水平及其与肺炎严重程度的相关性,以 及NGAL、MCP-1 水平预测重度肺炎的受试者工作特征(ROC)曲线。结果 重度肺炎患者NGAL 及 MCP-1 水平高于普通肺炎组和对照组(P <0.05)。NGAL 及MCP-1 与肺炎严重度指数评分呈正相关(r =0.701 和0.762,P =0.002 和0.008)。血清NGAL、MCP-1 水平及二者联合预测重症肺炎的ROC 曲线下面积分 别为0.715、0.744 和0.840。结论 重症肺炎患者血清NGAL 及MCP-1 水平与肺炎严重程度有关,且血清 NGAL 及MCP-1 水平可预测重症肺炎的发生。

Abstract:Objective To investigate the diagnostic value of neutrophil gelatinase-associated lipocalin (NGAL) and monocyte chemotactic protein-1 (MCP-1) in patients with severe pneumonia. Methods Forty patients with common community acquired pneumonia (common pneumonia group) , 49 patients with severe community acquired pneumonia (severe pneumonia group) and 55 healthy people were selected (control group). The levels of NGAL, MCP-1 and their correlation with the severity of pneumonia were measured. The ROC curves of NGAL and MCP-1 were used to predict the severity of severe pneumonia. Results The levels of NGAL and MCP-1 in severe pneumonia patients were higher than those in normal pneumonia group and control group(P < 0.05). NGAL and MCP-1 were positively correlated with PSI scores respectively (r = 0.701 and 0.762, P = 0.002 and 0.008). ROC curves of serum levels of NGAL, MCP-1 and both for predicting severe pneumonia were 0.715, 0.744 and 0.840, respectively. Conclusions The serum levels of NGAL and MCP-1 in patients with severe pneumonia were related to the severity of pneumonia, and the serum levels of NGAL and MCP-1 could predict the occurrence of severe pneumonia.
