大脑生物反馈治疗结合心理护理在促进老年躯体形式障碍患者康复中的临床效果

目的 探讨大脑生物反馈治疗结合心理护理在促进老年躯体形式障碍患者康复中的临床效果.方法 选择2011年1月～2012年5月滨州医学院烟台附属医院老年躯体形式障碍156例,分为对照组(给予单纯心理护理)和观察组(给予心理护理结合大脑生物反馈治疗),每组各78例.观察分析两组患者治疗前、治疗后第4、8周末SCL-90测评情况、护士观察量表(NOSIE)评分及临床疗效.结果 ①治疗前两组患者SCL-90评分差异无统计学意义(P＞0.05),两组治疗第4、8周在躯体化、强迫、焦虑、抑郁、精神病性等方面经比较差异有统计学意义(P＜0.05).②治疗后两组患者总积极分、总消极分和病情总评分情况均明显优于治疗前,差异有统计学意义(P＜0.05);而观察组的总积极分、总消极分和病情总评估评分情况在第4、8周末均明显优于对照组,差异有统计学意义(P＜0.05).③观察组总有效率89.74％明显高于对照组74.36％,差异有统计学意义(P＜0.05).结论 大脑生物反馈治疗结合心理护理能明显改善老年躯体形式障碍患者的躯体化、焦虑、抑郁、精神病性症状,改善人际敏感、敌对、偏执,提高疗效,促进康复.

Clinical effect of brain biofeedback combined with psychological nursing in the treatment of elderly patients with somatoform disorder

Objective To study the clinical effect of brain biofeedback combined with psychological nursing in the treatment of elderly patients with somatoform disorder. Methods The clinical data of 156 old patients with somatoform disorders from January 2011 to May 2012 in Yantai Affiliated Hospital of Binzhou Medical University were retrospec- tively analyzed, and divided into the control group （simple psychological nursing） and study group （psychological nurs- ing combined with brain biofeedback therapy） with 78 cases in each group. The SCL-90 evaluation and NOSIE score of two groups before treatment and 4, 8 weeks after treatment were compared, and the total effective rates of two groups were observed. Results ①The differences of SCL-90 evaluation in the two groups before the treatment were not statis- tically significant （P 〉 0.05）; the differences of somzatization, obsessive-compulsive symptom, dysphorla, depression, psychoticism in the two groups 4, 8 weeks after the treatment were statistically significant （P 〈 0.05）. ②The total posi- tive points, total negative points, the total condition score in the two groups after the treatment were better than those before the treatment, the differences were statistically significant （P 〈 0.05）; the total positive points, total negative points, the total condition score in study group 4, 8 weeks after the treatment were better than those in control group, the differences were statistically significant （P 〈 0.05）. ③In study, the total efficiency of 89.74% was significantly higher than that in control group 74.36%, the difference was statistically significant （P 〈 0.05）. Conclusion Psychological nursing combined with brain biofeedback treatment can significantly improve the elderly patients with somatoform dis- order somatization, anxiety, depression, psychotic symptoms, interpersonal sensitivity, hostility, paranoid, improve cura- tive effect, promote the rehabilitation.
