结节性甲状腺肿与甲状腺腺瘤的超声诊断对比分析

摘    要:目的:探讨甲状腺腺瘤及结节性甲状腺肿的超声声像图特征,以提高2种疾病鉴别诊断水平。方法:对比分析45例甲状腺腺瘤和50例结节性甲状腺肿患者的声像图特点,并与术后病理对照。结果:两组均见结节,其中结节性甲状腺肿结节以多发常见(32例,64.0%),无明显包膜。甲状腺腺结节以单发为主(29例,64.4%);两组结节周围组织回声比,差异有统计学意义(P〈0.05)。彩色多谱勒血液显像示:两组结节周边及内部血流信号差异有统计学意义(P〈0.05)。结论:彩色多普勒超声可提高结节性甲状腺肿和甲状腺腺瘤的鉴别诊断水平,具有重要的临床意义。

Abstract:Objective:To investigate the sonographic features of thyroid adenoma and nodular goiter and to improve the sonographic diagnosis.Methods:Sonographies of 45 patients with thyroid adenoma and 50 patients with nodular goiter confirmed by pathological examination were reviewed retrospectively;the sonographic characteristics were studied and compared with pathological diagnoses.Results:Nodule presented in both nodular goiter and thyroid adenoma,the numbers of nodules were usually multiple(32 cases,64.0%) in nodular goiter and solitary in adenoma(29 cases,64.4%),and capsule was not found outside the nodular goiter and might found outside the adenoma.The echogenicities around the nodules of the two diseases were different with significat difference(P<0.05).On CDFI,the distributions of blood flow signals in and around the two nodules were significantly different(P<0.05).Conclusion:Thyroid adenoma and nodular goiter have different features of Color Doppler ultrasographies,which are of important clinical significance in the differential diagnosis.
