糖尿病增加前列腺增生患者血尿发生率

目的探讨糖尿病（DM）对良性前列腺增生（BPH）患者血尿及BPH组织中血管新生的影响。方法分析295例单纯BPH及43例BPH合并DM患者的临床参数，应用免疫组化法检测其前列腺组织病理标本中血管内皮生长因子（VEGF）及CD34抗原的表达，并计算微血管密度（MVD）。结果BPH合并DM组患者血尿发生率显著高于单纯BPH组（P〈0．05）。合并DM的BPH组织中VEGF、MVD显著高于单纯BPH（P〈0．05和P〈0．01），并且VEGF与MVD呈正相关（Rs=0．152，P〈0．01）。结论血管新生是BPH的一个重要环节，并与BPH血尿的发生有密切关系。DM能够促进BPH组织中的血管新生，从而加速BPH的发生、发展过程，并造成BPH患者血尿发生率增加。

Diabetes mellitus increases hematuria incidence of patients with BPH

Objective To study the clinical significance and effect of diabetes mellitus(DM) on hematuria incidence of patients with benign prostatic hyperplasia(BPH). Methods Clinical data of 43 BPH with DM and 295 simple BPH patients from Jan 1994 to Apr 2003 in our hospital were analyzed retrospectively.The expression of vascular endothelial growth factor(VEGF) and CD34,microvessel density(MVD) were detected by immunohisochemical staining for 30 prostate tissues in each group.Results BPH with DM had more hematuria incidence than that of simple BPH(P=0.024).VEGF expression(P=0.02)and MVD(P<0.01)in BPH with DM group was significantly higher than that in simple BPH group,and there was a positive correlation between VEGF and MVD(Rs=0.152,P<0.01).Conclusion DM plays an important role in the occurrence and clinical out come of BPH by increasing the expression of VEGF and MVD.DM is responsible for higher hematuria incidence of patients with BPH.
