Doppler彩色血流成象在肝实质占位性病变诊断中的应用

对136例共164个肝实质占位病变进行Doppler彩色血流成象(DCFI)的研究,其中原发性肝癌99例,转移性肝癌7例,肝血管瘤25例,其他5例。研究表明肝内占位在DCFI上表现为内部及内缘出现彩色血流,可呈短线状、长线状、树枝状、簇状及“掌形”等。结果表明,DCFI对肝癌诊断的敏感性(98.33％)、特异性(95.45％)及准确性(97.56％)均明显高于单纯的二维超声(P<0.005),因此,认为DCFI可作为超声诊断肝占位性质的特异性手段。

APPLICATION OF DOPPLER COLOR FLOW IMAGING IN THE DIAGNOSIS OF THE SOLID HEPATIC LESIONS

One hundred thirty six patients with 164 space occupying lesions of liver, including 99 hepatocellular carcinomas (109 lesions), 7 metastatic tumors (9 lesions), 25 hemangiomas (41 lesions) and 5 others (5 lesions) were studied with Doppler Color Flow Imaging (DOFI). The diagnosis was confirmed by hepatic artery angiography in 23 patients, and by operation in the remaining patients. Color blood now in the internal or fringe of the hepatic lesion was shown on DCFI display. The manifestations of color flow were in the shape of "short-string", "long-string", "branch", "cluster" or "palm". The color flow detected with DOFI was found in 94.5% cases of hepatocellular carcinomas, 77.8% cases of metaetatic liver tumors and 17.1% cases of hemangiomas. DOFI is very useful in the differential diagnosis between malignant and benign lesions (including heraangioma). When DOFI was combined with conventional US. the sensitivity, specificity and accuracy in the diagnosis of liver cancer were 98.33%, 95.45% and 97.56% which were much higher than conventional US alone (P<0.005). Therefore, DOFI should be regarded as a specific method in the diagnosis of solid hepatic lesions.
