屋尘螨过敏的常年性变应性鼻炎患者皮肤试验同IgE的关系

目的 探讨对屋尘螨过敏的变应性鼻炎患者中变应原皮肤试验反应强度同血清特异性和总IgE水平之间的关系。方法 选择69例对屋尘螨过敏的散发变应性鼻炎患者为实验组，80例为健康对照组。对69例变应性鼻炎患者进行变应原皮肤试验和屋尘螨的特异性IgE水平的检测，所有个体均检测血清中总IgE水平。结果 实验组变应原皮肤试验反应强度同血清特异性IgE之间有相关性（r=0．379，P=0．001），血清总IgE水平同特异性IgE水平、变应原皮肤试验之间无明显相关性（P〉0.05），实验组血清总IgE的几何均数水平明显高于对照组（分别为224IU／ml和50IU／ml，P〈0．001）。结论变应原皮肤试验反应结果同血清屋尘螨的特异性IgE水平有相关性，在变应性鼻炎的诊断中有重要作用，变应性鼻炎患者血清总IgE水平明显高于健康对照组，其在疾病诊断中有参考价值，但通过血清总IgE水平对变应原皮肤试验结果和特异性IgE水平进行预测的意义不大。

The relationship between skin test responsiveness and IgE in allergic rhinitis patients sensitive to Dermataphagoides pteronyssinus

OBJECTIVE To clarify the relationship between skin test responsiveness, serum specific IgE and total IgE in Dermataphagoides pteronyssinus (Derp)induced allergic rhinitis patients. METHODS Sixty-nine Derp-sensitive allergic rhinitis patients and 80 nonatopic healthy controls were recruited for this study.The re-sponse to skin test and serum specific IgE levels in the 69 Derp-sensitive allergic rhinitis patients were mea-sured respectively. Serum levels of total IgE were de-termined in all subjects. RESULTS The correlation be-tween the skin test responsiveness and serum Derp specific IgE and total IgE levels in allergic rhinitis pa-tients were investigated. There was a correlation be-tween skin test responsiveness and serum Derp spe-cific IgE (r=0.379,P=0.001). The serum total IgE level did not correlate with Derp specific IgE levels and skin test responsiveness (P>0.05). The total serum IgE lev-els in allergic rhinitis patients were compared with those of the controls. The total serum IgE levels of the pa-tients were much higher than those of the controls (geometric mean of the patients and the controls were 224 IU/ml and 50 IU/ml respectively,P<0.001). CONCLUSION Skin test reactivity and the specific serum IgE levels are correlated. These two results are very important in the diagnosis of allergic rhinitis. The total serum IgE levels of the allergic rhinitis patients are much higher than those of the controls. There are some reference values for the serum total IgE levels to the diagnosis of the Derp-induced allergic rhinitis patients, but its value is limited to predicting the skin test results and the specific IgE value according the total IgE value.
