PPH与传统开放式外剥内扎术治疗重度环状混合痔的对比研究

摘    要:为比较吻合器痔上黏膜环切钉合术(PPH)与传统开放式外剥内扎术治疗重度环状混合痔的临床效果,将120例环状混合痔患者随机分为两组,各60例,一组采用PPH治疗(治疗组),一组采用传统开放式外剥内扎术治疗(对照组),对比分析两组患者的疗效。结果显示,治疗组手术时间、疼痛指数、住院时间、恢复工作时间、患者满意度、术后各种并发症的发生率及复发等情况均优于对照组,P〈0.05。结果表明,PPH操作简单、安全可靠、出血少、疼痛轻、并发症少、恢复快,远期效果肯定,是治疗重度环状混合痔的首选术式,但要注意病例选择、手术操作细节、并发症防治及与其他手术方式的配合运用。

A Comparative Study on the Therapeutic Efficacy of PPH and Open External Dissection and Internal Ligation in the Treatment of Severe Circumferential Mixed Hemorrhoids

Abstract:The objective of the study was to evaluate the therapeutic results of PPH and open external dissection & internal ligation(OEDIL) in the treatment of severe circumferential mixed hemorrhoids(SCMH). One hundred and twenty patinets with $CMH were randomized into two groups, patients in the study group underwent PPH( n =60) ,while patients in the control group OEDIL( n =60). As results,the aspects of operation time, pain index, hospital stay duration, recovery work time, patients satisfaction, occurrence of postoperative complications and recurrence were better in the study group than that in the control group( P 〈0.05). It is concluded that PPH is a procedure of first choice in the treatment of SCMH due to its simpler manipulation , safety, less bleeding, pain and complications, and quick recovery, ensure long-term therapeutic efficacy. While case choice, manipulation detail, prevention & treatment of complications and combination other procedure must be noticed.
