肛周脓肿30例脓液细菌培养和药敏分析

摘    要:为研究肛周脓肿菌群分布和抗生素应用的效果,对30例肛周脓肿行细菌培养加药敏试验。结果显示,大肠埃希菌24例,占75%(24/32),其他菌7例(肺炎克雷伯菌肺炎亚种3例,藤黄微球菌1例,表皮葡萄球菌1例,阴沟肠杆菌1例,奇异变形杆菌1例),感染两种细菌3例,无细菌生长2例。亚胺硫霉素、阿米卡星药物敏感度高,分别为23例和22例。氨苄西林和替卡西林耐药率高,分别为95.83%和87.50%。头孢类药物耐药率一般在29.17%~66.67%,左氧氟沙星耐药率为58.33%。结果表明,肛周脓肿多为混合细菌感染,以大肠埃希菌感染为主,抗生素普遍耐药,成脓后应尽早手术治疗。

The Analysis on the Pus Germiculture and Drug Sensitivity Test of 30 Patients with Perianal Abscess

Abstract:The aim of the study was to study the bacterial distribution in perianal abscess and the effect ot treatment with antibiotics. Oermiculture and drug sensitivity test were carried out in 30 cases with perianal abscess. Results showed culture bacterioflora of 30 cases include Escherichia coli,Klebsiella pneumoniae subsp, pneumoniae 3 cases, Micrococcus luteus 1 case, Staphylococcus epidermidis 1 case, Enterobacter cloacae 1 case and Proteus rnirabilis 1 case, among which Escherichia coli existed in 24 cases taking up 75 % of the total number (24/32),other bacterioflora of 7 cases. Three cases had two different culture bacterioflora while two cases had no bacteria. Imipenem and Amikacin' drug sensitivity was relatively high, which were active in 23 cases and 22 cases respectively. The resistance rate of Ampicillin and Ticarcillin were the highest,95.83% and 87.50% respectively, that of cephems was between 29.17 % to 66.67%, and that of Levofloxacin was 58. 33%. It is concluded that most patients with perianal abscess are infected by more than one bacteria, and Escherichia coli is in the majority. Antibiotics resistance exists generally. Once the pus comes into being surgical operations should be done as soon as possible.
