吻合器痔上黏膜环切术对肛门功能的影响

为比较吻合器痔上黏膜环切术（PPH）与Milligan—Morgan（MM）术治疗重度内痔对肛门功能的远期影响，回顾2004年5月至2007年4月96例重度内痔患者资料，其中42例采用PPH治疗（PPH组），54例采用MM术治疗（MM组），对比分析两组患者术后远期肛门功能。结果显示，PPH组肛门闭合、控便及感觉功能异常患者少于MM组，差异有统计学意义，P〈0．01。PPH组患者术前和术后3年肛门闭合、控便及感觉功能差异无统计学意义，P〉0．05；MM组术后3年存在肛门闭合、控便及感觉功能异常的患者多于手术前，差异有统计学意义，P〈0．01。结果表明，PPH治疗重度内痔对远期肛门功能影响小。

Influence of PPH on Postoperative Anal Function

The objective of the study was to compare the influence of PPH and Milligan-Morgan procedure （MMP） on anal function in the treatment of severe internal hemorrhoids. The data of 96 patients with severe hemorrhoids who were admitted from May 2004 to April 2007,were analized retrospectively. Of the 96 cases,42 had undergone PPH （the PPH group）,while 54 had undergone MMP（the MMP group）. As results, such complications as incomplete anal closure,incontinence and abnormal sensative function were less in PPH group than in MMP group（ P〈0.01）. And the aspects of anal closure, control of defecation and sensive function were statistically similar before and 3 years after operation in the PPH group（ P 〈0.05） ;while that were statistically different before and 3 years after operation in the MMP group（ P 〈0. 01）. It is concluded that PPH has smaller long-term impact on anal function in patients with severe internal hemorrhoids.
