| 摘 要： | 目的 探讨直肠癌术后局部复发的预防和治疗问题.方法 回顾性分析1997年1月至2007年12月所收治的46例直肠癌术后局部复发患者的临床资料.结果 保肛手术后局部复发34例,占73.91%.共40例行再次手术治疗,其中36例行复发肿瘤切除,随访33例,1 年生存率77.78%,3年生存率44.44%,5年生存率13.89%. 结论慎重选择保肛手术,根治术遵循直肠全系膜切除术(TME)原则是减少复发的两个主要方面,术后的密切随访可早期发现肿瘤复发.明确直肠癌术后局部复发后应争取手术治疗并进行以手术为主的综合治疗仍可取得较好的治疗效果,晚期患者放疗可缓解症状.
