缺血性胸痛对冠状动脉病变结果的影响分析

摘    要:目的了解缺血性胸痛对冠状动脉血管病变结果的影响。方法2004年9月至2008年2月连续164例因胸痛行冠状动脉造影患者,依据是否为临床典型心绞痛或心肌梗死所表现的特征性胸痛(缺血性胸痛),分为:72例典型心绞痛或心肌梗死(I组)和92例因胸痛临床疑似冠心病(Ⅱ组),以计算机辅助定量冠脉造影观察2组在对冠状动脉病变。结果①两组在血管直径狭窄程度≥50%例数分别为:70例vs.8例,P<0.01。②两组的单支病变血管数分别为:24例vs.8例,P<0.01。另I组的双支病变14例,三支病变12例;而Ⅱ组则无双支病变或三支病变。③两组的病变血管数分别为:88支vs.8支,P<0.01。④I组和Ⅱ组的局限性病变分别为:50处vs.6处,P<0.01;管状病变分别为:18处vs.2处,P<0.01。结论缺血性胸痛仍是诊断冠心病的重要临床依据,而且能反映出冠状动脉血管病变狭窄的程度、对病变血管的数量、形态、范围有影响,可作为冠状动脉血管病变的独立预测变量。

Abstract:Objective To observe influence ischemic chest pain on coronary artery lesions. Methods From Sep.2004 to Feb. 2008, 164 consecutive cases with chest pain undergone coronary artery angiography was divided into two groups: 72 case AP or MI(group Ⅰ) and 92 cases suspected of CAD for chest pain (group Ⅱ ), coronary artery lesions were assessed by quantitative coronary angiography (QCA). Results The numbers of coronary vessel stenosis more than 50%, single vessel lesions, total coronary segments of culprit lesions and lesion specific characteristics(discrete, tubular lesion) of culprit lesions of group Ⅰ were significantly more than that of group Ⅱ. Conclusion Ischemia cheat pain was not only more important symptom for diagmosing of coronary heart disease,but effecting on numbers of coronary vessel stenosis, segments of culprit lesions, and severity and extent of morphology of culprit lesions also, was an independent predictor of coronary artery disease.
