三叉神经痛手术治疗——附174例报告

目的：为提高三叉神经痛手术治疗效果，减少术后复发率，方法：对１７４例原发性三叉神经痛采用口腔内手术径路，行三叉神经高位切断术＋神经周围分支切除术，结果：控痛率１００％，术后复发４例（２．３％）无严重并发症，结论：三叉神经高位切断术口内之径路，方法新，创伤小，恢复快，效果好，神经切断，高位比低位好，多支比单支好，切断＋部分切除比单纯切断好。

Current situation and prospect of surgical treatment for trigeminal neuralgia

Objective: To improve the curative effect of surgical treatment for trigeminal neuralgia and reduce the recurrence rates. Method: The high level rhizotomy and partial branches rhizotomy via intraoral approach had been used in 174 cases suffered from trigeminal neuralgia. Results: Rate of pain control was 100% and recurrence rate of pain was 2.3%. There were no serious complications after operation. Conclusion: The intraoral approach for the high level rhizotomy of trigeminal nerve is a new method with little trauma quick rehabilitation and good effect. We think that 1. the rhizotomy performed at a high level is better than that at the low level, 2. sectioning of multiple branches is more effective than cutting a single branch, 3. cutting and removing a segment of nerve tissue might be less recurrent than simple rhiziotomy procedure.
