超声定位下颈内静脉置管在急危重症肾功能衰竭患者血液净化治疗中的临床应用

目的探讨超声定位下颈内静脉置管在急危重症肾功能衰竭患者血液净化治疗中的临床价值。方法将收集的98例急危重肾功能衰竭患者作为本次的研究对象,将患者随机分为观察组和对照组,每组49例,观察组患者采用超声定位下颈内静脉置管,对照组患采用传统的颈内静脉置管,比较两组临床效果。结果观察组患者的一次穿刺成功率、留置导管时间、并发症发生率及血液净化治疗时血液流量不足率均明显优于对照组(p<0.05),组间比较差异有统计学意义。结论超声定位下颈内静脉置管在急危重症肾功能衰竭患者血液净化治疗中应用可取得较好的效果,且该穿刺静脉置管术还具有操作简单、导管留置时间长、并发症少等优点,临床价值较高,应推广应用。

CLINICAL ANALYSIS OF ULTRASONIC POSITIONING UNDER THE INTERNAL JUGULAR VEIN CATHETER IN BLOOD PURIFICATION TREATMENT PATIENTS WITH SEVERE RE- NAL FAILURE

Objective To investigate the ultrasound localization of internal jugular vein catheterization in critically ill patients with renal failure clinical value in the treatment of blood purification. Methods Collected 98 cases of acute and severe renal failure patients as the research object in this time, the patients were randomly divided into observation group and control group, 49 cases in each group. The observation group were treated by ultrasound guided cannulation of the internal jugular vein ; The control group patients were treated with internal jugular vein cath- eterization. The clinical effect was compared between the two groups. Results The success rate of observation, in- dwelling catheter time, complieation rate and blood purification treatment when the blood flow insufficiency rate are significantly better than the control group a puncture group patients （p 〈 O. 05 ）. There are statistically significant differences between groups. Conclusion Ultrasound localization of internal jugular vein catheterization in critically ill patients with renal failure in blood purification treatment can achieve good results, and the puncture vein catheter- ization also has the advantages of simple operation; long time of indwelling catheter has the advantages of less compli- cations, etc. There is high clinical application value, and it should be popularized and applied.
