深圳市观澜地区产超广谱β-内酰胺酶菌株基因分型及耐药性分析

摘    要:目的:了解深圳观澜地区产超广谱β-内酰胺酶菌株基因型分布情况,并分析其耐药性。方法:利用PCR技术扩增产ESBLs菌株质粒DNA的基因,统计分析各基因型阳性率。应用琼脂稀释法检测所有菌株的最低抑菌浓度(MIC)。结果:81株基因分型,SHV型检出率为34.6%,TEM型检出率为9.9%,CTX-M-1型检出率为11.1%,CTX-M-9型检出率为44.4%。药敏试验结果显示亚胺培南MIC90为0.25μg/ml,敏感率为100%。阿米卡星和哌拉西林/他唑巴坦,MIC90分别为8μg/ml和16μg/ml,敏感率为86.1%和90.1%。结论:深圳观澜地区流行产ESBLs菌株主要为CTX-M型,其次为SHV型。亚胺培南对ESBLs菌株抗菌活性最强,其次为阿米卡星和哌拉西林/他唑巴坦,氨苄西林/舒巴坦抗菌活性差。

Analysis on gene type of strain producing ESBLs and drug resistance in Shenzhen Guanlan district

Abstract:Objective To understand the genotype distribution of the strain producing extend spectrum β-lactamases and to analyze drug resistance of which in Shenzhen Guanlan district.Method ESBLs genotypes were detected by polymerase chain reaction(PCR) and the positive rate was analyzed by statistics.MIC of the strains producing ESBLs were detected by agar dilution method.Results The detection rate of the strains producing SHV enzyme was 34.6%,which of the strains producing TEM enzyme was 9.9%,which of the strains producing CTX-M-1 type enzyme was 11.1%,which of the strains producing CTX-M-9 type enzyme was 44.4%.The MIC90 of imipenem was 0.25 μg/ml and the sensitiveness to drug of which was 100%.The MIC90 of amikacin and piperacillin-tazobactam were differently 8 μg/ml and 16 μg/ml and the sensitiveness to drug of which was differently 86.1% and 90.1%.Conclusion The main gene type of ESBLs is CTX-M type and the next gene type is SHV type.These strains producing ESBLs exhibits high level of susceptibility to imipenem,amikacin,piperacillin-tazobactam.The antibacterial activity of ampicillin-sulbactam is inferior.
