序贯疗法根除幽门螺杆菌疗效评价

目的:比较10 d序贯疗法与7 d三联疗法根除幽门螺杆菌(H.pylori)的疗效及安全性。方法:选取经胃镜检查确诊为有明显异常的慢性胃炎和消化性溃疡,且经胃镜下取黏膜作快速尿素酶试验,发现Hp阳性的患者77例,并将其随机分组,A组序贯疗法组(39例),前5天给予奥美拉唑、阿莫西林,后5天给予奥美拉唑、克拉霉素、左氧氟沙星;7 d三联疗法组(38例)给予奥美拉唑、阿莫西林、克拉霉素,疗程7 d。疗程结束后1个月,复查胃镜并取黏膜行快速尿素酶试验,阴性者为根除。比较两组患者Hp根除率及不良事件发生率。结果:两组患者Hp根除率分别为92.3%(36/39)和73.7%(28/38),差异有统计学意义(P<0.05)。结论:10 d序贯疗法根除Hp疗效较7 d疗法好,具有疗效高、不良反应低、依从性好的特点。

Effect assessment with sequential therapy for eradicating helicobacter pylori

Objective To assess the eradication and safety of a 10-day sequential regimen compared with conventional 7-day triple therapies.Methods Select diagnosed by endoscopy as having obvious abnormalities of chronic gastritis and peptic ulcer,and the mucosa obtained by endoscopy for rapid urease test,Hp-positive patients found in 77 cases and randomly grouped,sequential therapy group（39） 5 d before giving omeprazole,amoxicillin,5 d after giving omeprazole,clarithromycin,levofloxacin;7 d triple therapy group（37） given omeprazole,amoxicillin and clarithromycin,the course of treatment 7 d.One month after the end of treatment and repeated endoscopy mucosal taken by rapid urease test,negative for the eradication.Hp eradication rates were compared in patients and incidence of adverse events.Results Hp eradication rate of two groups of patients were 92.3%（36/39） and 73.7%（28/38）,the difference was statistically significant（P0.05）.Conclusion 10 d efficacy of sequential therapy compared with 7 d Hp eradication therapy is good,with high efficacy,low adverse events,the characteristics of good compliance.
