摘    要:目的:探讨足月妊娠脐带绕颈对分娩方式和围生儿的影响。方法:回顾性分析80例足月妊娠脐带绕颈病例为观察组,另随机选取80例足月妊娠非脐带绕颈病例作为对照组,分析比较两组孕妇胎儿宫内窘迫、新生儿窒息的发生率及脐带绕颈对围生儿及分娩方式的影响。结果:观察组较对照组胎儿宫内窘迫、新生儿窒息发生率高,产程延长,Apgar评分降低,脐静脉呈现乏氧性酸中毒(P<0.05);绕颈周数越多,胎儿窘迫、窒息发生率逐渐增加。结论:脐带绕颈会明显增加胎儿宫内窘迫、新生儿窒息及死亡的发生率。加强产前诊断和产时监护,选择最佳方式及时机分娩是降低围生儿死亡率及新生儿窒息率的有效措施。

Effect of circulor of umbilical cord on delivery ways and perinatal infants

Abstract:Objective To assess the effect of circulor of umbilical cord on delivery ways and perinatal infants in pregnancy at term.Method Eighty cases with circulor of umbilical cord in pregnancy at term were assigned into the treatment group,and 80 cases without circulor of umbilical cord in pregnancy at term as the control group.The effects of incidences of fetal distress and neonatal asphyxia,circulor of umbilical cord on perinatal infants and delivery ways of the pregnancies in the two groups were analyzed.Results The incidences of fetal distress and neonatal asphyxia,the length of first stage labor in the treatment group were evidently higher than in the control group(P〈0.05).Apgar score,acidosis of umbilical vein in the treatment group were obviously lower than that in the control group(P〈0.05).Conclusion Circulor of umbilical cord can evidently increase the incidences of fetal distress,neonatal asphyxia and death.Strengthening diagnosis and monitoring before birth,selecting proper delivery methods are effective measures of reducing the incidences of fetal distress and neonatal asphyxia.
