老年人低位直肠癌前切除术后回肠置管造口与横结肠造口的对比研究

目的:探讨和比较老年人低位直肠癌前切除术后(LAR)实行回肠置管造口与横结肠造口的临床效果。方法:选取65例老年低位直肠癌患者,33例LAR术后行回肠置管造口术为置管组,32例LAR术后行横结肠造口术为造口组,比较两组术后吻合口瘘发生情况及住院时间。结果:置管组1例(3.03%)发生吻合口漏,造口组1例(3.13%)发生吻合口漏,置管组平均住院时间为(12.0±3.5)d,造口组平均住院时间为(13.0±3.8)d,两组吻合口瘘发生率及住院时间比较差异无统计学意义(P>0.05)。结论:回肠置管造口术对老年人低位直肠癌LAR术后发生吻合口漏的预防作用与横结肠造口术比较,差异无统计学意义(P>0.05),且提高了患者晚期生活质量,具有可行性。

Comparison of ileum indwelling catheter neostomy and transverse colostomy for elderly patients with low rectal cancer after LAR

Objective To discuss and compare the clinical effect of ileum indwelling catheter neostomy and transverse colostomy for elderly patients with low rectal cancer after LAR.Method Total 65 elderly patients with low rectal cancer after LAR were randomly divided into two groups,33 cases received ileum indwelling catheter neostomy as the catheter group,32 cases received transverse colostomy as the neostomy group.The incidence of stoma fistula and hospital stay were compared between the two groups.Results In the catheter group,1 case（3.03%） had stoma fistula,the mean hospital stay was（12.0±3.5） d;in the neostomy group,1 case（3.13%） had stoma fistula,the mean hospital stay was（13.0±3.8） d.The incidence of stoma fistula and hospital stay between the two groups had no significant differences（P〉0.05）.Conclusion The preventive role of ileum indwelling catheter neostomy for low rectal cancer after LAR has no difference with transverse colostomy,and can improve the life quality,with feasibility.
