对胸水行CYFRA_(21-1)与CEA的联合检测

目的 探讨细胞角质蛋白19(CYFRA_(21-1))与癌胚抗原(CEA)在结核性胸水与癌性胸水中的诊断价值。方法胸水患者108例,其中癌性68例,结核性40例,分别测定其血清、胸水中CYFRA_(21-1)和CEA的浓度。结果①两种肿瘤标记物浓度在恶性胸水中明显高于结核性胸水;②癌性患者胸水中CYFRA_(21-1)的浓度明显高于血清中的浓度,而胸水中CEA浓度与血清中浓度的差异无显著性;③胸水CYFRA_(21-1)的检测存在一定的假阳性。结论 联合检测CYFRA_(21-1)与CEA对结核性与恶性胸水的诊断有较高的临床价值。

Combining the evaluations of CYFRA_(21-1) and CEA in differentiating malignant pleural effusions from the benign ones

to discuss the role of tumour marker in differentiating malignant pleural effusion from benign ones. Methods Pleural and serum levels of cytokeratin 19 fragment (CYFRA21-1) and carcinoembryonic antigen (CEA) were determined in 108 patients with pleural effusions(68 malignant and 40 tuberculous). Results Pleural and serum CYFRA21-1 and CEA levels were significantly higher in the cancer group than that in the tuberculous group. In the cancer group, CYFRA21-1 levels in pleural fluid did not differ significantly from that in the serum. CYFRA21-1 levels showed some false positive reaction in the pleural fluid of the patients with tuberculous effusions. Conclusion Combining the CEA test with CYFRA21-1 analysis appears to be very useful in differentiating malignant pleural effusion from the benign ones.
