乳腺癌伴锁骨上淋巴结转移的临床综合治疗分析

目的探讨锁骨上淋巴结转移乳腺癌的综合治疗效果。方法共收治2l例患者，其中9例为同期乳腺癌伴锁骨上淋巴结转移，12例为乳腺癌术后锁骨上转移。均给予新辅助化疗＋手术＋术后放疗、化疗或／和内分泌治疗。结果2l例患者V区淋巴结均阳性，且大部分位于颈后三角区。随访2～4年，均无颈部局部复发，4例一年半死于肺、肝及骨转移，余健在。结论结合新辅助化疗最大限度的切除原发灶及腋窝和锁骨上淋巴结等综合治疗，对改善患者的预后具有积极的作用。

Combined Therapy of Breast Cancer with Supraclavicular Lymph Node Metastasis

Objective To explore the effect of neo-adjuvant chemotherapy combined systemic therapy on breast cancer patients with supraclavicular lymph node metastases. Methods Twenty-one breast cancer patients with supraclavicular lymph node metastases（ nine cases of metastases found at diagnosis, twelve metastases found after adenomammectomy） received neoadjuvant chemotherapy, surgery and combined-modality therapy. Results In all patients,lymph nodes in V region were positive. The most of them was located in posterior cervical triangle. All this patients were followed up for 2 to 4 years, four of them died of pulmonary metastasis, hepatic metastasis and bony metastasis in a half past one year. The others were survival now. Conclusions Neo-ad- juvant chemotherapy plus resection of the primary tumors and axillary and supraclavicular lymph nodes maximally can improve the prognosis.
