联合应用Sysmex UF-100尿流式细胞仪、干化学分析仪及尿沉渣镜检检测尿液中红细胞、白细胞和管型

摘    要:目的:比较Sysmex UF-100尿流式细胞仪,干化学分析仪及尿沉渣镜检三种方法对尿液中红细胞(RBC)、白细胞(WBC)和管型(CAST)检测的敏感性及其影响因素。方法:对3000例病人的尿液分别用UF-100自动尿液分析仪、干化学分析仪及尿沉渣镜检三种方法进行分析,并将三种方法对RBC、WBC、CAST的检测敏感性进行比较。结果:UF-100检测出RBC、WBC、CAST的阳性率分别为:37.8%、38.8%、13.7%;干化学法检测出RBC、WBC的阳性率分别为:33.4%、30.3%;尿沉渣镜检检测出RBC、WBC、CAST的阳性率分别为:31.5%、35.4%、4.3%。UF-100检测结果偏高,假阳性率偏高。结论:UF-100尿流式细胞仪和干化学分析仪不能取代尿沉渣镜检,建议三种方法联合应用减少检验误差,提高尿液分析质量。

Abstract:Objective: To compare the sensitivity and influencing factors in detecting urine erythrocyte, leukocyte and casts by using Urine flow cytometer Sysmex UF-100, test-strip measurement and sediment microscopy. Methods: Urine specimens of 3000 inpatients were collected and every sample was examined by UF-100, test-strip and microscopy, then comparing the sensitivity of these three urine analysis methods. Results: Analyzed by UF-100, the positive rate of urine erythrocyte, leukocyte and casts were 37.8%, 38.8% and 13.7% respectively; Analyzed by test-strip, the positive rate of urine erythrocyte and leukocyte were 33.4% and 30.3% respectively; Analyzed by sediment microscopy, the positive percentage of urine erythrocyte, leukocyte and casts were 31.5%,35.4% and 4.3% respectively. The positive rate and false positive rate of UF-100 was higher compared with test-strip and sediment microscopy. Conclusions: Sediment microscopy can not be replaced by Urine flow cytometer Sysmex UF-100 and test-strip in urine analysis. Combining these three methods would be optimal to improve the accuracy and precision in urine analysis.
