左卡尼汀对透析中低血压患者微炎症和血压的影响

目的观察左卡尼汀对存在透析中低血压的维持性血液透析患者微炎症和血压的干预作用。方法选择透析中低血压患者22例,随机分为治疗组12例和对照组10例,每次透析结束后,治疗组给予静脉注射左卡尼汀2 g,对照组注射等量的生理盐水。治疗前后检测2组患者血清炎症标志物C反应蛋白(CRP)、白细胞介素-6(IL-6)和肿瘤坏死因子α(TNF-α)水平,比较血液透析中血压的变化和透析中低血压发生次数。结果与治疗前相比,治疗组CRP、IL-6和TNF-α水平显著下降,透析中血压明显升高,透析中低血压发生率明显降低;上述指标组间比较均有显著性差异(P均<0.05)。结论左卡尼汀可明显改善存在透析中低血压的维持性血液透析患者的微炎症和血压,可作为一种长期治疗药物。

Effects of levocarnitine on microinflammation and blood pressure in intradialytic hypotension patients

Objective It is to observe the interference effects of levocarnitine on microinflammation and blood pressure in maintenance hemodialysis(MHD) patients with intradialytic hypotension.Methods 22 MHD patients with intradialytic hypotension were randomly divided into two groups: treatment group(n=12) and control group(n=10).In the treatment group 2g of levocarnitine was infused to the patient at the end of each dialysis treatment,while normal saline was given to the control group.In each group the serum level of C-reactive protein(CRP),interlukin-6(IL-6),tumor necrosis factor alpha(TNF-α) were measured before and after treatment.The changes of blood pressure in hemodialysis and the frequency of intradialytic hypotension were compared.Results Compared with that before treatment,the level of CRP,IL-6,TNF-α were significantly reduced(P<0.05) in treatment group,while the blood pressure was significantly increased(P<0.05),the frequency of intradialytic hypotension was significantly reduced.There were also significant differences in the indexes above between the two groups(all P<0.05).Conclusion Levocarnitine can obviously improve microinflammation and blood pressure in MHD patients with intradialytic hypotension,it can be used as a long term therapy.
