食管平滑肌瘤124例分析

本文收集了国内近30年来文献报告的食管平滑肌瘤114例,连同本院10例共124例,进行了临床分析。男多于女,男女之比为2.9:1。约80％小于40岁。间歇性吞咽困难占56％,胸骨后疼痛占38％,上腹疼、胃灼热占21.8％。肿瘤以中段居多(60.7％)。单发117例,多发7例。本文分析了肿瘤的生长方式,描述了X线钡餐和内镜检查时的诊断要点和注意事项。粘膜外肿瘤摘除加肌层修补术是目前简便、安全、创伤小的理想术式。

Leiomyomata of the Esophagus.An Analysis of 124 Collected Cases

In reviewing the literature of our country in the last thirty years 124 cases of esophageal leiomyomata, including 10 surgically removed lesions in our hospital, were collected. Although esophageal leiomyoma is the most common of the benign tumors of the esophagus, it is still rare as compared with carcinoma. It occurs more often in men than women, by a ratio of 2.9:1. The most frequent symptoms are dysphagia, retrosternal pain and pyrosis. The best diagnostic means are esophagograph and en- doscopic examinations of the esophagus. Diagnostic problems may arise, as the small lesion of this disease may mimic esophageal carcinoma, mediastinal neoplasms, polyp etc. Transthoracic enucleation of the tumor is the treatment of choice. Approximately 10% of patients with leiomyoma of the esophagus require partial esophageal resection. Postoperatiye morbidity is minimal and results are excellent.
