后路内固定辅助复位加360°融合术治疗腰椎滑脱症

【目的】总结应用后路内固定辅助复位加360°融合术治疗腰椎滑脱症的效果。【方法】2001年3月至2002年8月,使用后路切开椎弓根钉固定系统辅助复位加360°椎间融合术治疗成人峡部裂性腰椎滑脱症23例,其中Ⅰ度滑脱7例、Ⅱ度16例,均伴有腰椎不稳症,或不同程度椎管狭窄症和神经症状。【结果】随访3～20个月,平均13个月,滑脱椎体复位率87％,随访8个月以上者15人,14人获骨性融合。临床疗效依侯树勋等制定标准,优13例,良8例,可2例,差0例,优良率91％。【结论】后路内固定辅助复位加360°融合术是治疗有手术指征的成人峡部裂性腰椎滑脱症的极佳方法。

Posterior Instrumented Reduction and 360 Degree Fusion for Lumbar Spondylolisthesis

[Objective] To summarize the results of posterior instrumented reduction and 360 degree fusion for lumbar spondylolisthesis . [Methods] 23 patients who suffered from adult lumbar isth-mic spondylolisthesis underwent operation using posterior instrumented reduction and 360 degree fusion from March, 2001 to August, 2002. According to Meyerding classification, 7 patients had degree I and 16 patients had degree II spondylolisthesis. All of them had lumbar instability, and various degree of spinal canal stenosis or neural symptoms. [Results] All of them were followed up for 3 to 20 months, averaged 13 months. The reduction rate of spondylolisthesis was 87% . Of 15 cases who were followed up for more than 8 months, 14 cases obtained osseous fusion. Based on Hou Shuxun Criterion for therapeutical evaluation, the efficacy was as follows: excellent: 13 cases, good: 8 cases, acceptable: 2 cases, bad: 0case. The rate of excellent and good was 91% . [ Conclusion ] Posterior instrumented reduction and 360 degree fusion is an excellent choice for adult lumbar isthmic spondylolisthesis with indication for operation.
