涎腺超声评分联合弹性成像对干燥综合征疾病活动的评估价值

摘    要:目的:探究涎腺超声评分联合弹性成像对于干燥综合征(sjogren syndrome,SS)疾病活动情况的评估价值。方法:收集我院风湿科门诊或病房诊治SS患者80例,所有患者均符合2016年美国风湿病学会(American College of Rheumatology,ACR)/欧洲风湿病联盟(European League Against Rheumatism,EULAR)的分类标准。根据干燥综合征疾病活动指数(EULAR sjogren Syndrome Disease Activity Index,ESSDAI)将患者分为疾病活动情况活动组及稳定组。对两组患者腮腺、颌下腺进行0-4分超声评分与弹性成像检查。绘制两种方式及联合使用的受试者工作特征曲线(receiver operating curve,ROC),计算曲线下面积(area under curve,AUC)分析比较其评估价值。结果:单独使用超声评分判断SS疾病活动情况的ROC曲线下面积为0.668;单独使用腮腺、颌下腺弹性成像判断SS疾病活动情况的ROC曲线下面积分别为0.774、0.692;联合使用两种方式评估SS疾病活动情况的ROC曲线下面积为0.828,此时的灵敏度为81.3%,特异度为81.2%。结论:涎腺超声评分系统联合弹性成像技术有助于SS不同疾病活动情况的评估。

Abstract:Objective: To explore the value of salivary gland ultrasound score combined with elastography in evaluating disease activity of sjogren syndrome (SS). Methods:A total of 80 cases of SS diagnosed and treated in the outpatient department or ward of Rheumatology in our hospital were collected. All patients met the classification criteria of American College of Rheumatology (ACR)/ European League Against Rheumatism (EULAR) in 2016. Patients were divided into disease activity group and stable group according to Eular Sjogren''s syndrome disease activity index (ESSDAI). The parotid and submandibular glands of the patients in the two groups were scored with 0-4 ultrasound and examined with elastography. The receiver operating characteristic curve (ROC) of the two methods and their combination were drawn and the area under curve (AUC) was calculated to compare their evaluation values. Results: The area under ROC curve of using ultrasound score alone to judge disease activity of SS was 0.668. The areas under the ROC curve of using elastography of parotid gland and submandibular gland alone to judge the disease activity of SS were 0.774 and 0.692, respectively. The area under the ROC curve for the combined use of the two methods to assess disease activity in SS was 0.828, giving a sensitivity of 81.3% and a specificity of 81.2%. Conclusion: The salivary gland ultrasonic scoring system combined with elastography is helpful for the evaluation of SS in different disease activities.
