盐酸利托君在前置胎盘治疗中的有效性和不良反应分析

摘    要:目的探讨盐酸利托君治疗前置胎盘的有效性和不良反应。方法选择2010年4月至2012年8月在我院妇科治疗的前置胎盘患者76例,将其随机分为观察组和对照组,各38例。观察组予盐酸利托君治疗,对照组予硫酸镁治疗。对比观察用药后两组保胎效果、产后出血、围产儿结局和不良反应。结果观察组保胎成功率、妊娠期≥36周比率、延长孕周均明显优于对照组,差异有统计学意义94.74%VS.78.95%,71.05%VS.47.37%,(4.58±1.64)周VS.(3.22±1.39)周,P均〈0.051;两组产后出血率、新生儿体重和新生儿窒息率比较,差异无统计学意义39.48%vs.34.21%,(2694.37±336.85)gvs.(2571.08±554.92)g,5.26%VS.2.63%,P〉0.05]。两组不良反应率比较,差异无统计学意义(P〉0.05)。结论盐酸利托君抑制宫缩效果优于硫酸镁,其控制出血效果好,改善了妊娠结局,且安全性高,是治疗前置胎盘的一种有效药物。

Abstract:Objective To explore the clinical efficacy and adverse reactions of ritodrine hydrochloride for placenta praevia. Methods A total of 76 patients with placenta praevia admitted to our hospital from April 2012 to August 2012 were randomly divided into a study group and a control group, 38 for each group. The study group received ritodrine hydrochloride and the control group was treated with magnesium sulfate. The effi- cacy for abortion prevention, postpartum hemorrhage, perinatal outcomes, and adverse reactions were analyzed and compared between the two groups after medication uses. Results The success rate of abortion prevention and the ratio of gestational age greater than 36 weeks were significantly higher and prolonged gestational age was markedly longer in the study group than in the control group 94.74% vs. 78.95% ,71.05% vs. 47.37%, and (4.58 _+ 1.64)weeks vs. (3.22 + 1.39 )week; P 〈 0.05 for all comparisons]. There were no significant differences in postpartum hemorrhage volume, average birth weights, and morbidity of asphyxia in newborns between the two groups 39.48% vs. 34.21%, (2694.37 + 336.85)g vs. (2571.08 + 554.92)g, and 5.26 % vs. 2.63% ; P〉 0.05]. The rate of adverse reactions did not differ significantly between the two groups. Conclusions Ritodrine hydrochloride is better than magnesium sulfate in inhibiting uterine contraction. It has advantage of improving pregnancy outcomes and is an effective drug in treating placenta praevia with mild adverse reactions.
