手术患者记忆障碍与术中失血量关联性及护理干预措施

目的探究在手术过程中的失血量与患者出现记忆障碍的关联性及护理干预措施。方法截取我院2009年3月至2012年1月共68例行手术治疗患者,按术中出血量分为少量组（〈300m1）及大量组（≥300m1）,对比两组患者问临床资料差异性。结果大量组较少量组障碍发生率高29．12％,其中大量组轻、中、重度障碍分别为23．68％、26．32％、15．79％,少量组为16．67％、16．67％、3．33％,两组差异行统计分析提示P〈0．05,差异具显著性。结论手术过程中的失血量多少与患者POCD发生率明显正相关,在术中护理人员需积极配合医师,提高自身操作技术,避免患者低体温,全面、及时、有效的护理争取降低患者POCD发生。

The correlation of memory impairment with intraoperative blood loss in surgical patients and the nursing interventions

Objective To explore the correlation of blood loss with memory impairment in patients during surgical process and the related nursing interventions. Methods 68 patients who had received surgical treatment during the period of March 2009 to January 2012 were divided into small amount group （ 〈 300 ml） and large amount group （ ~〉 300 ml） according to the amount of intraoperative bleeding. The differences in clinical data were compared between the two groups. Results The rate of memory impairment was 29.12% higher in large amount group than in small amount group. The rate of impairment was 23.68% for slight impairment, 26.32% for moderate impairment, and 15.79% for severe impairment in large amount group and 16.67%, 16.67%, and 3.33% in small amount group, respectively （ P 〈 0.05 ）. Conclusions The amount of blood loss is markedly as- sociated with o the incidence of postoperative memory impairment. Nursing staff should actively cooperate with surgeons and improve their professional skills to avoid occurrence of low body temperature in patients during surgery. Comprehensive, timely, effective nursing care can lower incidence of postoperative memory impair- ment.
