老年人髋部骨折的医疗费用及影响因素分析

目的调查韶关市50岁以上髋部骨折住院病人的医疗费用，分析影响因素，为合理控制费用提供依据。方法收集广东省韶关市14家医院股骨头骨折、股骨颈骨折、股骨粗隆间骨折的病例，对医疗费用及其影响因素进行统计学分析。结果髋部骨折的平均住院时间为21天，平均住院费用为12184元，住院费用在2003至2006年间有增加趋势。影响医疗费用的因素有住院时间、骨折类型、医院级别、患者城乡类别。结论通过提高医院管理水平、选择适宜治疗手段、落实双向转诊、完善医疗保障水平来控制住院病人费用，减轻病人负担，提高健康水平。

Analysis on influencing factors of medical expense in elderly patients with hip fracture

Objective To investigate medical expense of the hip fracture patients aged over 50 years in Shaoguan, and to analyze the related influencing factors, and provide evidences for reasonably controlling the medical expenses. Methods We screened patients diagnosed as hip fracture on admission in Shaoguan from 14 hospitals to be identified and included in the study. Results The mean length of stay in the hospitals was 21 days. The average medical expense was RMB 12184. There was a growing tendency from 2003 to 2006 in the medical expense, and the influencing factors were the mean length of stay in the hospitals, the types of the frac-ture, the class of the hospital and the regional types. Conclusion The medical expense could be controlled by improving hospital management level, selecting proper therapy methods, implementing two-way referral system and improving medical security level to reduce the patients＇ burden and increase their health conditions.
