CT三维多平面重建在鼻腔及鼻旁窦疾病的应用(附55例手术、病理报告)

目的探讨CT三维多平面重建图像(MPR)在鼻腔、鼻窦中的应用价值。方法粘膜囊肿4例,慢性鼻窦炎鼻息肉45例,鼻腔鼻窦肿瘤6例,所有病例均用多平面重建冠状、矢状、水平成像,必要时任意方向重建。所有病例均经手术、病理证实。结果多平面重建图像与常规冠状扫描图像相当,比常规轴扫图像,对窦口-鼻道复合体、筛板等正常解剖结构,肿瘤、鼻息肉、囊肿的位置、大小、形态及周围组织关系显示更清楚,更符合医生观察习惯。结论螺旋CT三维多平面重建图像显示窦口-鼻道复合体及筛板正常解剖结构及变异,鼻腔鼻窦病变准确,有助于临床医师纤维内窥镜手术方案的制订和实施。

Application of Three-dimensional Multi Planar Reconstruction (3-D MPR) Imaging of the Nose and Paranasal Sinuses by Means of Helical CT

Objective To evaluate the clinical application of three-dimensional multi planar reconstruction (3-D MPR) imaging of the nose and paranasal sinuses by means of helical CT.Methods There are multi coronary,sagital and transverse axial planar reconstruction in the 4 cases with abcess ,45 cases with chronic parasinusitis and nasal polyp , 6 cases with tumors of the nose and parasinuses. If necessary, we can also reconstruct the imaging of any other planar.All of the cases were confirmed by operation and pathology.Results The imaging of multi planar reconstruction is as clear as one of common coronary scanning.Compared with the imaging of common transverse axial planar,MPR imaging could display normal anatomical structure of ostiomeatal complex and cribriform plate,tumor,nasal polyp and abcess.MPR imaging could help clinical doctors observe it. Conclusion Three-dimensional multi planar reconstruction imaging could demonstrate more data in displaying normal or abnormal anatomical structure of ostiomeatal complex and cribriform plate, and lesions of the nose and paranasal sinuses.It is helpful for clinical doctors to cure by nasal endoscopic surgery.
