妊娠期高血压疾病对围产期结局的影响

目的:探讨不同程度妊娠期高血压疾病对围产期结局的影响。方法:对南京医科大学附属南京市妇幼保健院产科2004年1月~2006年9月113例妊娠期高血压疾病产妇进行回顾性队列研究,分组比较母婴结局。结果:妊娠期高血压(≥160/110 mmHg)组不良妊娠结局发生率明显高于轻度子痫前期以及正常血压组和妊娠期高血压(≥140/90 mmHg,<160/110mmHg)组(P<0.05)。结论:高血压与妊娠期高血压疾病的病情及围产儿预后密切相关。不仅可作为临床监测指标,而且对诊断和治疗以及改善母婴预后均有十分重要的意义。

Analysis of perinatal outcomes determined by hypertensive disorders complicating pregnancy

Objective:To investigate the effect of gestational hypertension with or without proteinuria on perinatal outcomes.Methods:A total of 113 cases with hypertensive disorders complicating pregnancy admitted into Nanjing Maternal and Children Health Care Hospital affiliated to Nanjing Medical University from Jan.2004 to Sep.2006 were divided into group A normotensive/gestational hypertension(≥140/90 mmHg,<160/110 mmHg)];group B gestational hypertension(≥160/110 mmHg)];group C(mild preeclampsia) and group D(severe preeclampsia).Results:Women who developed severe gestational hypertension had higher rates of both preterm delivery and small-for-gestational-age infants than women with mild preeclampsia or mild gestational hypertension.Conclusion:In women who have hypertensive disorders complicating pregnancy,increased rates of preterm delivery and delivery of small-for-gestational-age infants are present in those with severe hypertension.Blood pressure can be used as marker for clinical monitoring and is important for diagnosis and treatment of hypertensive disorders complicating pregnancy and for improving the perinatal outcomes.
